Fourth-Quarter 2025 Data Posted; Reports Updated

The AIS Data Team has posted new data to the subscriber dashboard Online Search Tool and In-App Download Enrollment Data Spreadsheets. This data primarily reflects fourth-quarter 2025 status but includes first-quarter 2026 data for Medicare Advantage (MA),** dual-eligibles and some Medicaid membership.

Most related in-app downloads and special reports now reflect the most recent data available. Keep an eye out in the coming weeks for a detailed analysis of changes from the second quarter of 2026.

For this update, we added six MCOs, all of which serve the MA or dual-eligible market:

PACE of Cincinnati (#796), a PACE plan serving Ohio;
LIFE COORDINATED Commonwealth PACE (#2090), a PACE plan serving Kentucky;
Senior CommUnity Care of Maryland (#2091), a PACE plan serving Maryland;
Buckeye PACE (#2093), a PACE plan serving Ohio;
Elite Health Plan (#2094), a California-based MA plan; and
Cherry Blossom PACE (#2095), a PACE plan serving Virginia.

We renamed four MCOs due to consolidations or rebrandings:

(#1881) Abilis Health (fka Signature Advantage)
(#1694) ElderServe Health (fka RiverSpring Health)
(#1511) One Senior Care (fka LIFE Northwestern Pennsylvania)
(#1393) VieCare LIFE (fka Lutheran SeniorLife LIFE Programs)

Notable Enrollment Changes

Decreases in the Medicaid space were once again larger than last quarter: Medicaid HMO membership fell by more than 1M, and Medicaid FFS decreased by nearly 296K lives since the third-quarter update.

Notable insurer-level changes in the managed Medicaid space since the last update include the following:

• Decreases in Florida, Minnesota and California fueled many of the losses, in addition to an overall reduction in membership.
UnitedHealthcare (#1263) lost the most members nationally, more than 113K, due to reductions in several states.
CareSource (#257) gained more than 109K Medicaid members, largely due to a contract pick-up in Nevada.

Public exchange figures decreased nearly 847K in the fourth quarter, as part of an overall decline in the individual market. Note: These figures represent 4Q2025; expected decreases for 2026 will be reflected in the 1Q2026 update.

The following insurers saw the highest shifts in the public exchange space:

Centene (#1543), the largest marketplace insurer, dropped the most members, more than 385K, representing 6% of its ACA membership.
UnitedHealthcare (#1263) also lost members, roughly 159K, in the fourth quarter.
Aetna (#1359), which exited the public exchange market for 2026, lost more than 127K.

Commercial group risk enrollment again decreased since the third quarter, dropping 203K members, fueled this time by losses in the small group market (461K). ASO/self-funded plan membership offset losses somewhat, adding 84K members since last quarter.

Notable changes in commercial group include:

Blue Cross and Blue Shield of Texas (#508) lost nearly 175K group risk members.
Blue Cross and Blue Shield of North Carolina (#126) lost 54K group members.
• On the flip side, Cigna Corporation (#1224) continued its focus on Commercial markets, gaining 85K ASO members.

After a turbulent open enrollment season where many large payers retracted their offerings, Medicare Advantage (MA) and duals plans gained 270K members; this is the only sector that entirely contains 2026 membership data for this release. Medicare FFS membership, which lags MA and is as of December 2025, increased by 178K.

Notable payer changes include:

• After enacting an aggressive strategy for 2026, Humana gained more than 1.1M MA members and 156K duals since the last update, by far the largest increase.
Devoted Health (#1947) continued to be the dominant smaller payer, gaining nearly 238K MA members, a whopping 142% increase over last year.
• After large gains for 2025, UnitedHealthcare and Elevance Health both left unprofitable markets for 2026, losing 834K and 314K, respectively.
